Dairy Intolerance in Perimenopause

Have you noticed that dairy doesn’t feel quite right anymore? Maybe your morning latte causes bloating, or yoghurt leaves you feeling gassy.

Dairy intolerance in perimenopause is a real issue, and it’s more common than you might think.

Hormonal Changes Can Affect Digestion

During perimenopause, estrogen and progesterone levels begin to fluctuate and eventually decline. While these hormones are best known for their role in reproductive health, they also influence digestion and gut function.

Changes in hormone levels can affect:

  • Gut motility (how quickly food moves through the digestive tract)
  • The balance of bacteria within the gut microbiome
  • Inflammation and gut sensitivity

As a result, some women become more sensitive to foods they previously tolerated without any issues, including dairy products.

We Naturally Produce Less Lactase With Age

Lactose is the natural sugar found in milk and many dairy products. To digest lactose, the body relies on an enzyme called lactase.

For many people, lactase production gradually declines with age. This means that dairy products that were once well tolerated can start causing symptoms such as:

  • Bloating
  • Gas
  • Stomach cramps
  • Loose stools or diarrhoea

This age-related decline is one of the most common reasons dairy becomes harder to digest during midlife.

Stress Can Make Digestive Symptoms Worse

Perimenopause is also a time when many women experience higher levels of stress, poorer sleep, and increased cortisol production.

Chronic stress can influence digestion by:

  • Slowing or disrupting digestive processes
  • Altering gut bacteria
  • Increasing gut sensitivity

As a result, foods that were previously well tolerated may suddenly trigger symptoms.

Is It Lactose or Casein?

Not all dairy reactions are caused by lactose.

Type of SensitivityWhat It AffectsCommon Symptoms
Lactose intoleranceMilk sugar (lactose)Bloating, gas, diarrhoea
Casein sensitivityMilk protein (casein)Digestive discomfort, congestion, skin flare-ups, fatigue

Some women find that switching to lactose-free products helps significantly. Others continue to experience symptoms, which may suggest sensitivity to the proteins found in dairy rather than the lactose itself.

What Can You Do?

You don’t necessarily need to eliminate dairy completely. The key is finding what works best for your body.

Strategies to Try

  • Choose lower-lactose options such as hard cheeses, Greek yoghurt, or lactose-free milk.
  • Try fermented dairy products like kefir and live-culture yoghurt, which are often easier to digest.
  • Support gut health through adequate fibre intake, stress management, regular exercise, and a varied diet.
  • Consider a lactase enzyme supplement if lactose appears to be the issue.
  • Experiment with calcium-fortified alternatives such as soy, almond, or oat milk if dairy continues to cause symptoms.

Don’t Forget About Calcium and Protein

Dairy remains a convenient source of both calcium and high-quality protein, two nutrients that become increasingly important during perimenopause for maintaining bone density and lean muscle mass.

If you’re reducing or avoiding dairy, make sure you’re replacing these nutrients through other foods or supplements where appropriate.

Bottom Line

The good news is that you don’t necessarily need to give up dairy completely.

With a little experimentation, many women can find dairy products that work well for them while still supporting their bone health, muscle mass, and overall well-being.
